Week 1 Complete: Second Dose Tonight

One week results with tirzepatide - 2.7kg lost and preparing for the second injection. Honest assessment of appetite changes, exercise integration, and what to expect going forward.

Current Weight: 126.3kg (-3.7kg from start)
Energy Level: 8/10
Hunger Level: 4/10 (returning but manageable)
Sleep: Consistent 7-8 hours
Today: Second injection scheduled for 8 PM

The Numbers Don’t Lie

It’s September 4th, 11 AM, exactly one week since I started Mounjaro. When I made the decision, I weighed 130kg. The day I went to the doctor, I was 129kg. Today: 126.3kg.

That’s 2.7kg (almost 6 lbs) in one week. It feels brutal - in the best way possible.

How This Week Played Out

Days 1-3 (Fri-Sun): Peak medication effect. Could barely finish one KnowEats meal per day, supplemented with a can of tuna before bed. My eating window was typically 2-3 PM for the main meal, late evening for the tuna.

Days 4-7 (Mon-Thu): As the medication wears off, I can now eat two full plates daily. Still no excessive hunger, but I have actual appetite now rather than forcing myself to eat.

Exercise routine: Monday gym session, Tuesday swimming, today swimming again, Friday planning another gym session. The activity is definitely helping.

Second Dose Tonight

Tonight at 8 PM, I take my second injection. I understand GLP-1 medications are depot drugs - they’re stored in fat tissue and released slowly. The first days have the strongest effect as your body adjusts, then it tapers until the next dose.

Even with this tapering, I haven’t returned to my old hunger levels. I could comfortably eat two plates daily now, but that’s still a massive reduction from before.

Calorie Math

My basal metabolic rate is around 2,200-2,300 calories. On training days, I can burn up to 3,000 total. Even eating two full KnowEats meals (~1,400-1,500 calories max), I’m running a deficit of 1,500-2,000 calories daily.

This level of deficit explains the rapid initial weight loss.

Side Effects Check

Stress-related: Had a work stress spike this week, which triggered my usual seborrheic dermatitis flare-up. That’s my normal stress response, not Mounjaro-related.

Medication-related:

  • Increased thirst (drinking ~3 liters water daily)
  • Heavy feeling if I eat too much or too greasy
  • Better hydration overall

No negative side effects from the medication itself. The increased water intake actually feels beneficial.

Goal Reality Check

At this rate of 1-2kg weekly loss, I could reach my initial goal of 110kg much sooner than expected. My ultimate targets:

  • First milestone: 110kg
  • Ideal range: 90-100kg

I don’t want to go too low - no interest in looking gaunt. The goal is finding where I look and feel my best.

What I’ve Learned

The medication works, but you still have to be disciplined. When I feel real stomach hunger, I eat. When I don’t, I don’t force it. The appetite suppression is real, but it’s not magic - you still need to make conscious choices.

Most importantly: this doesn’t feel like traditional dieting. There’s no constant battle with willpower or food obsession. It’s just… quieter.
